For the weight loss benefits, you need pre-authorisation.  Contact Keyhealth at 0860 671 060 to obtain authorization.  You would have to do a Screening test by doing  a Health Assessment at a clinic.  The assessment form can be obtained from any pharmacy or downloaded from healthbooster. 

The results of the screening test can be submitted by faxing it to 012 679 4471. 

For all beneficiaries when the Health Assessment BMI is ≥ 30:

If you qualified and got authorisation from Keyhealth, you may contact us to book an appointment.  Keyhealth would pay for 3 consultations with the dietitian.  If you lost more than 3% of your weight within the 3 weeks, you may request authorization for another 3 consultations.

One biokineticist consultation and a follow-up consultation would be covered by Keyhealth.  The biokineticist would create a home exercise programme for you.
